Insurance Coverage

Many lactation visits are covered at 100% by the following plans:

• United Healthcare/UMR

• Aetna

• Cigna

• Colorado Medicaid

• First Health

• Curative

Who Can Book Immediately

If you have United/UMR, Aetna, or Curative your visit is typically covered at 100%.
You can book your appointment without verification.
Your only out-of-pocket cost is the travel fee for in-home visits.

Who Needs to Verify Coverage

If you have Cigna or First Health, please complete the appropriate coverage verification first by clicking the verification button.
Once verified, you may book your appointment.

Kaiser Insurance

I am happy to bill Kaiser for covered lactation services. However, insurance coverage and payment cannot be guaranteed. If Kaiser denies or does not reimburse your visit, you are responsible for the balance at my current self-pay rates.

Please note that travel fees apply for all in-home visits are not covered by insurance and are the responsibility of the client, regardless of insurance coverage.

Travel Fees

Card on File Policy: A valid credit card is required prior to all in-home visits and all self-pay office and virtual visits. Your card will only be charged for your patient responsibility, including applicable deductibles, coinsurance, denied insurance claims, travel fees, self-pay balances, or cancellation fees in accordance with our office policies.

In-Home Visit Travel Fee
In-home visits are typically available within approximately 15 miles of my Lafayette office. A flat $50 travel fee applies to in-home visits within this service area.

Need support farther away? Extended travel may be available based on distance and scheduling, with an additional travel fee.

Colorado Medicaid clients: Medicaid-covered visits must be provided in-office or virtually, as travel fees cannot be billed for in-home visits.

Colorado Medicaid

Colorado Medicaid visits are covered at 100% and must be scheduled in-office.
A referral is required from the provider linked to the active Medicaid coverage (either the lactating parent’s provider or the baby’s provider). The referral must include the provider’s NPI number and signature, and can be faxed to 720-677-0263.
